Hearth Care and EOLProfessionals produce short summaries of practical information formatted as infographics, borrowing from the Sketchnote style of recording visual information.
Sketchnotes are made using visual cues such as simple drawings or doodles. Information is usually recorded by a 'Sketchnoter' for future reference while listening to a lecture or presentation.
Hearth Care and EOLProfessionals have 'flipped' this concept and process to provide notes on end-of-life care to health care professionals, carers and patients, so as to introduce"point-of-care" education, with the assistance of social media.
Hearth Care aims to provide customised, person-to-person, end-of-life education to patients, carers, health care professionals, support personnel and their local communities, delivered at the point of care.
Hearth Care believes that inclusive, experiential learning, delivered in warm, simple language to those directly involved in care "on the ground", can affect cultural change around death and dying.
To this end, Hearth Care offers a series of workshops (half day; full day; and 2 days in length) for patients, carers and volunteers, and for each discipline of healthcare professional.
Hearth Care provides
1. Supportive and palliative care consultation services to patients at their homes, in clinics, in aged care facilities, and in private hospitals;
2. Education consultations to hospital, community health and aged care facilities; and
3. Presentations and/or education sessions to community groups.
